At Palo Alto Oral & Maxillofacial Surgery, we make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this goal, we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.

Accepted Insurance

We are in network with Delta Dental.

We are out of network with all other insurances. This means that we would need to charge upfront for all services on the day of the appointment, though as a courtesy we would still send the claims to the patient’s insurance. For non-Delta claims, dental insurances are asked to send responses, including any payments, directly to the patient.

We ask that patients with non-Delta insurances fill out this form with their complete insurance information since this will be required for claim submission. While we can attempt to submit claims with partial information, insurances are likely to discard them as incomplete rather than processing them.

We are excluded from Medicare, meaning that neither the office nor patients can bill Medicare for treatment at this office.
